Summer Registration

The following is our summer registration form. We ask that you please fill out the required fields before submitting. Following your submission, we will contact you to discuss payment options.

Purchase Policy
1. By completing the registration, the registering party is committing to the full cost of the program.
1.1. Various payment options are available for your convenience only; changes will not be made to the predetermined payment options.
1.1.1. The monthly payment option is broken down into an initial payment inclusive of the first and last months of the chosen program, followed by a number of consecutive payments appropriate to the duration of the chosen program. The initial payment will not reflect the same amount as the eight consecutive payments and will also contain the annual non-refundable registration fee of $25.00 CAD.
1.1.2. The monthly payment option is not a monthly subscription with the availability for anytime cancellation, rather it is the full cost of the program divided for monthly payments.
1.1.3. Financial adjustments may be applied for late registration, at the discretion of Russian Classical Ballet administration.
1.1.4. There are no financial adjustments for holidays, conflicting extracurricular activities, and vacations that result in non-attendance or withdrawal from competitions and/or the annual performance.
1.2. Charge for insufficient checks is $25.00 CAD.
2. The purchase of services, programs and products can be made via e-transfer, cash or check payment.

Refund Policy
1. There are generally no refunds after a party has registered for a program, event, activity, etc. with the Russian Classical Ballet Academy.
1.1. Instructors at the Russian Classical Ballet Academy are subject to change throughout annual programs, substitute and/or new instructors are not chosen at the discretion of clients participating in and/or paying for annual programs. Clients are prohibited from breaking contract on the basis of instructor preference.
1.2. In the case of medical condition and/or accident to the party registered to attend dance program(s), written statement from a physician and a written/signed notice of discontinuance must be issued to the Russian Classical Ballet Academy by the same person(s) who are registered or their parent(s)/guardian(s).
1.2.1. Absence from programs, classes, activities, events, etc. without a physician’s statement does not constitute as notice and will not be sufficient for reimbursement.
1.2.2. In cases of debilitating medical condition, serious injury and/or accident to the party registered to attend the program(s), refund will be issued only for the period of time that the registered party will be unable to attend. The portion of the payment made for attended classes/events will not be refunded.
1.2.3. For person(s) under the age of 18 registered by parent(s)/guardian(s), medical conditions, serious injuries and/or accidents of the parent(s)/guardian(s) responsible for program payment will not result in a refund. Refunds will only be issued in the event that the person(s) attending the program is/are medically unfit to continue their participation.
3. Considering the young age of certain registered parties, the parent/guardian(s) who have registered for the Toddler's Ballet Program will have the privilege to observe their child's progress for a month to decide whether they are satisfied with their child's ability to continue with the program. In the case of program discontinuation, a full refund will be issued with the exception of the annual registration fee of $25.00 CAD and the initial payment for the cost of the program which includes the first and last month of the program.
4. New clients who have registered for membership, may try two non-refundable classes. In the event that the client(s) wishes to discontinue, the remainder of the paid membership fees will be refunded.

Terms of Service
It is essential that each person(s) participating in programs, activities, events, and performances with/at the Russian Classical Ballet Academy is familiar with and abides by our basic rules of conduct and policies to ensure that:

1. All staff are able to perform their professional duties unimpeded.

2. All staff/members are able to establish a respectful and amicable environment.

3. Learning experience is maximized without disruption or intervention.

Registration
1. By registering with the Russian Classical Ballet Academy, the registering party is committed to their program of choice; one cannot switch to a different program mid-year.
1.1. Changes to the program of choice may be made the following registration year.
1.1.1. In the case of a medical condition or emergency, with written statement from a physician, changes to the choice program may be modified at the discretion of the Russian Classical Ballet Academy.
1.1.1.1. If the registered party(s) in question is permitted to change programs to accommodate medical condition or emergency and, in the event, that the new program price differs from the original paid, unused funds will be allocated as store/program credit for future use. No refunds will be issued.
1.1.1.2. In the event that the Russian Classical Ballet Academy closes its business, all store and program credit and program payments will not be refunded.
1.2. If the registering party is interested in making additions to their program, (ex. Extra classes, private classes, etc.) they may do so throughout the year.
1.2.1. Payments for private classes are made a month in advance and must be pre-arranged based on instructor availability.
1.2.2. Cancellations or schedule changes to private classes must be given 24-hour notice, otherwise the registered party(s) will be charged for the missed class in full with the addition of a $20 no-show fee.
2. For registering parties under the age of 18, a parent/guardian(s) must consent to registration and provide their information upon registration.
3. Attendance of programs, classes, events, etc. prior to registration completion and payment confirmation will not be tolerated.

Proper Conduct
1. Instructors reserve the right to remove any student from classes/programs, if that student:
1.1. Refuses to respect and abide by the direction of the instructor(s) (permanent or substitute) in charge of the class/activity/event, etc.
1.2. Is exhibiting rude or aggressive behavior.
1.3. Refuses to adhere to the established dance wear dress code.
2. If at any point, a parent/guardian(s) interferes/disrupts program activities/classes/rehearsal, etc. demanding a staff member's immediate attention, while displaying a lack of respect and concern for students and staff, the Russian Classical Ballet Academy reserves the right to remove the associated student from classes/programs.
2.1. Aggressive and/or rude behavior will not be tolerated.
2.2. All concerns/complaints must be made out in writing and directed to the Russian Classical Ballet Academy via email or mail. Approaching the receptionist or instructors, who are not qualified to assist with concerns regarding administrative policies or decisions regarding the removal of the attending party from participation, will not be tolerated.
2.3. Parent(s)/Guardian(s) wishing to request an audience may do so by phone or email to schedule an appointment.
2.4. Contacting instructors personally, through phone, email or in person, to discuss any matters regarding registration, classes, administration etc. is strictly forbidden. Please direct all questions via our general email at general@balletrus.com or phone at +1 (647) 268-5400. Parent(s)/Guardian(s) or registered persons(s) who are unwilling to adhere to the aforementioned clause will be disqualified from the registered program, banned from studio attendance, and will be responsible for paying the full cost of the program by the end of registered year.
3. Students are expected to come to classes, rehearsals, events, etc. on-time and prepared with proper dance wear, grooming, and equipment.
3.1. If a student exhibits poor or late attendance, except in the case of special circumstances, inappropriate dance wear and/or grooming, the instructor reserves the right to decide whether said student may participate in, observe, or attend the class/rehearsal/event/activity.
4. In order to minimize program interruptions, students are encouraged to use the restroom prior to the beginning of their class/rehearsal.
5. Parent(s)/Guardian(s) will not be permitted to enter/sit within the classroom during lessons/rehearsals. Those who wish to see their children during their class are invited to join our open class at the end of the year.
6. No food or drinks, other than water are permitted in the studio.
6.1. Students are expected to attend programs with their own water bottles. No glass bottles are permitted for safety reasons.
7. Students are only permitted to change in the designated changing area or prior to their arrival at the studio.
8. Notices may be handed out in class if considered necessary by staff. It is the responsibility of the parent(s)/guardian(s) of the attending party to be vigilant of any notices received.
9. Students who do not maintain attendance throughout the year, and especially in the two months prior to the annual performance, will not be permitted to participate in the annual performance. Please try to avoid scheduling conflicts from April – June.

Liability Waiver
I/we realize that participation in dance classes and activities could result in some possible personal injury. Despite precautions being taken by the studio, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces associated with the Russian Classical Ballet Academy. I/we agree to release from responsibility the Russian Classical Ballet Academy including management, staff, students, partnering companies and facilities hosts from any cause of action, claims, or demands now and in the future. I/we will not hold Russian Classical Ballet Academy liable for any personal injury including scrapes, bruises, cuts, sprains, fractures, broken bones, concussions, death or any personal property damage/loss, which may occur on the premises or associated facilities before, during or after classes, performances, and/or events.
